1. Have you ever applied for services with our office before?

Yes	If you answered yes, please contact your caseworker at the Family Support Division.
No	If you answered no, proceed to the next question.

2. Are you currently receiving medical assistance or a cash grant, or have you received a cash grant from the Nevada State Welfare Division in the past?

Yes	If you answered yes, contact Nevada State Welfare Division's Child Support Unit at 486-8500 for assistance in filing a case.
No	If you answered no, proceed to the next question.

3. Do you have an order, such as a divorce decree, paternity judgment, temporary protective order, etc., that says you are to receive child support?

Yes	If you answered yes, fill in the following blanks and click on the "Request Application for Child Support" button to initiate your request: Enter the abbreviation for the state where the non-custodial parent lives. Enter the abbreviation for the state that issued or registered the order.
No	If you answered no, proceed to the next question.

4. Were the mother and father of the child for whom you are seeking child support married at the time the child was born, or is the father's name on the Nevada birth certificate if the child was born on or after 10/01/97?

Yes	If you answered yes, fill in the following blank and click on the "Request Application for Child Support" button to initiate your request: Enter the abbreviation for the state where the non-custodial parent lives.
No	If you answered no, fill in the following blank and click on the "Request Application for Child Support" button to initiate your request: Enter the abbreviation for the state where the child was conceived.
